We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in ensuring the safe and effective use of medications for our patients. This position requires strong clinical skills, attention to detail, and the ability to work collaboratively within a multidisciplinary team.MICROMERCHANT / LONG TERM CARE EXPERIENCE / SKILLED NURSING /DutiesReview and interpret medication orders to ensure accuracy and appropriateness.Provide clinical pharmacy services, including medication therapy management and patient counseling.Collaborate with healthcare providers to optimize patient care plans.Monitor patient outcomes and adjust medication regimens as necessary.Participate in the development of pharmacy protocols and guidelines.Conduct medication reconciliation during patient admissions and discharges.Educate patients on proper medication usage, potential side effects, and interactions.Maintain accurate records of medications dispensed and ensure compliance with regulatory standards.Assist in training pharmacy interns or technicians as needed.
