Job Title: Registered Nurse (RN) - Home Health Job Specialty: Maximus NYIAP Job Duration: 260 days Shift: Day - 5x8 - 9am - 5pm Guaranteed Hours: 40 per week Experience: Minimum two (2) years in a HOME setting within the last fifteen (15) years License: Current RN Licensure in New York State Certifications: BLS (required before start date) Must-Have: Recent experience conducting comprehensive assessments of patients Job Description: - Perform initial in-home pre-evaluation of patients, evaluating health status, strengths, care needs, and preferences to guide individualized care plans. - Review consumer medical documentation and health referral forms as necessary. - Enter evaluation data into electronic form and transmit as required. - Document any concerns, conflicting information, or issues during evaluation. - Discuss health care options, supports needed, service vendors, and waiver options with consumers. - Initiate communication between Call Center and eligible consumers for enrollment in managed care health plans. - Collaborate with interdisciplinary care team members to improve care coordination and facilitate service delivery. - Collect quality review data and required documentation for outcome measurements. - Identify opportunities for health promotion and illness prevention. - Maintain knowledge of community resources, payor requirements, and network services. - Meet all standards outlined in annual performance criteria and bonus template. - Perform other duties as assigned by Regional Nurse Manager or project management.
