Job Description:
We are searching for a devoted RN case manager to coordinate and
advocate for our patients' care. To this end, case management role
focused on discharge planning, with additional information on
certifications, skills, and job responsibilities. Job Role and
Responsibilities Discharge Planning : You will primarily focus on
discharge planning, which includes coordinating patient discharges
to home, community settings, or skilled nursing facilities
(SNF).You will handle tasks such as arranging for transportation,
obtaining housing, and referrals for specific services (e.g.,
hemodialysis or wound vacs). Collaborative Work : You'll work as
part of an interdisciplinary team alongside social workers and
physicians. You will need to be able to function independently
while contributing to team discussions and rounds. Caseload : The
caseload can vary based on the number of case managers and social
workers available on the floor. Clinical Knowledge : Expected to
have knowledge in discharge planning, including referrals to
different care facilities and services. Knowledge of tools like
CarePort, MCG, or InterQual is preferred but not mandatory. Work
Environment Unit Type : You will typically work in Med-Surg or
Telemetry units, which involves a variety of patient care
scenarios. Shift Schedule : Standard Monday to Friday, 8:00 AM to
4:30 PM shifts, with 7.5 hours of work and a 1-hour lunch break.
Dress Code : Business casual or scrubs of any color except royal
blue or maroon (neutral colors are preferred). A lab coat is
provided. Skills and Experience Required Experience : You must have
at least one year of inpatient case management experience. Prior
experience in critical care is preferred but not mandatory. Key
Skills : You need to demonstrate critical thinking, time
management, and the ability to handle a moderate to heavy caseload
efficiently. Being able to work independently while maintaining a
positive attitude and collaborating effectively with the team is
crucial. Support and Resources Department-Specific Orientation :
New hires will undergo a 2-day hospital orientation that includes
training on the EMR system. Additionally, there will be 1-2 weeks
of preceptor-guided training, depending on the individual's ability
to learn and perform required tasks. Supportive Environment :
Management supports taking a full lunch break and ensures that
breaks follow facility policies. Additional Information Utilization
Review (UR) : This position is focused on discharge planning, and
UR, which handles insurance approvals and transportation
arrangements, is handled by a separate department. Common Patient
Types : You will manage patients requiring discharge to home, SNFs,
or community settings, and may also need to assist with specific
care arrangements like wound care or dialysis. Requirements : BLS
